• Information necessary about child to ensure health and safety protocols

• Information to ensure regulatory requirements

• Up-to-date Immunization Records

About the Child and Family:

• Helps us understand each child we enroll and prepare to make them feel welcome

• Plans or Assessments (Care plan/IEP/ASQ-3) 

Required State Forms (WA):

• Washington state requires licensed childcare facilities to collect certain information and retain records about children/families we serve

Registration Fee + Tuition​

• $100 Registration Fee

• Set a start date + Contract

• Tuition paid prior to service
